Roles and Responsibilities

The Lytton First Nations Health Department is entrusted with promoting the well being and physical health of all community members from newborns to the Elderly. In order to deliver various and complex services, the Lytton First Nations Health Department is organized into three interconnected programming areas:


Community Wellness Programs, Home and Community Care Programs, and Community Health Programs. 


The Health Department plays a pivotal role in the planning and delivery of other health-related services including the programs and services delivered by Nzenman Child and Family Development Centre, health services delivered by Interior Health Authority, services offered to Elders through the Lytton First Nations Seniors Complex, counselling and support services offered through the Nlaka'pamux Health and Healing Society and various Contracted services. 


  

PATIENT TRAVEL 


Tl’Kemtsin Health Centre provides assistance with travel needs within the Lytton First Nation community. 


For many of our people, especially those living in rural or remote areas, it can be challenging to access necessary medical care. We offer support to help with these challenges through our patient travel office located at the Tl’Kemtsin Health Centre. 

Dependent on the situation, we can assist with the cost of transportation, meals, and sometimes- accommodation to help travel to a medical appointment outside our community. This support is intended as a supplement and does not cover the full cost of travel.


You can access​ patient travel support if:

  • ​Medical services are covered by MSP or a public health agency.
  • Services eligible under the Health Benefits Program (e.g., dental, vision, specialists etc.)
  • ​​Treatment at an FNHA funded facility for substance use (formerly NNADAP).


Patient travel helps with travel to the closest appropriate provider, using the most efficient transportation appropriate for the need and medical condition.  


How can you request assistance? 


There are two forms that are required to be filled out in full:


The Request For Patient Travel form needs to be filled out and returned to patienttravel@lfn.band before the appointment date.


The Confirming of Attendance form must be printed and taken with you to your appointment so the health care provider can confirm you made it to your appointment. This form then is to be returned to  patienttravel@lfn.band as confirmation that the appointment was attended.
